Pair Team is a California medical group, founded in 2019, that provides Enhanced Care Management (ECM) - a benefit covered by Medi-Cal. Each member is matched with a personal Care Manager who works with them one-on-one to build a plan for their medical, mental health, and everyday needs like housing and food.
